Referrals

In some cases, insurance carriers require a referral from a physician. We work closely with all of our primary care offices that can make a direct referral to Raleigh Neurology on your behalf. Once the referral is received, our team will contact you to schedule the appointment and start the journey.

Insurance plans can be confusing. Our skilled patient service team can help you understand your coverage. We participate with most major insurance carriers.

We are happy to assist you. In order to accept and process your request, we need a signed medical release form.

Help with External Forms

There are times when you may have an external form (examples include DMV, FLMA, etc.) that you need completed by a provider. We have a variety of ways you can provide these to RNA so we can coordinate completing them for you. Please note there may be a charge for some forms to be processed.
